HOW TO PLACE ACCOUNTS WITH USA CAPITAL CREDIT:

I.
Complete our "CLient Profile Sheet" (See below)
II.
If you have BAD CHECK ACCOUNTS you have two options:
A.) Mail us the original checks and the completed "Client Profile Sheet" or:
B.) If you prefer not to send the original checks just make us a copy of the front and back of
EACH check that you want to place with us and mail them to us with the "Client Profile Sheet"
III.
If you have BAD DEBT ACCOUNTS you also have two options:
A.) Mail us the original invoices, or whatever supporting documentation of the debt that you have, or:
B.) fill out the Client Profile Sheet and one Account Placement Form (see below) for EACH debt account that you want to place with us! (Be sure you have supporting documentation for the debt, such as promissory notes, invoices, etc. in case the customer requests supporting documentation)
IV.
Include as much contact information as possible on the consumer who incurred the debt!
V.
Our rates depend on the amount of initial placement with us,
By submitting this form client authorizes USA Capital Credit to begin immediate collection activity against the customer(s) placed with this form. Client agrees that any payments received after the date of authorization will be deemed as monies collected by USA Capital Credit and agrees to pay any and all applicable fees associated with such collection, and all subsequent collections, as outlined in the body of this agreement, to USA Capital Credit no later than 21 days following the actual collection. Client further agrees to place only those accounts that client has the legal authority to place.
